ALABAMA

PEARL--This program is designed to help older home-bound people learn new ways to manage their emotional health and improve their quality of life. If you are 65 years of age or older and live in Alabama, you may be eligible for free services. PEARL provides trained professional staff to work with you individually or with you and a caregiver.

MEDICARE

Medicare is our country's basic health insurance program for people 65 or older and many people with disabilities. DO NOT CONFUSE Medicare with Medicaid.

Medicare is a federal program that comes under social security.

Medicaid is a joint federal and state health care program that helps pay medical costs for people with low income and limited resources. It is usually run by state social service agencies. People generally qualify for one or the other, and sometimes both.

If you have received disability benefits from Social Security for two years, you are automatcally enrolled in Medicare. Medicare has two parts--hospital insurance and medical insurance. Hospital insurance helps pay hospital bills and some follow-up care. The taxes you paid while you were working finances this coverage, so it's free. The other part of Medicare, medical insurance, helps pay doctor's bills and other services. You will pay a monthly premium for this coverage if you want it. If you get Medicare and have low income and few resources, your state may pay your Medicare premiums and in some cases, other "out-of-pocket- expenses. To find out if you qualify, contact your state or local welfare office or Medicaid agency.

MILITARY

Uniformed services retirees, including retired National Guard members and reservists Medicare-eligible family members and widow/widowers certain former spouses if they were eligible for TRICARE before age 65.

Free or low-cost health insurance is available now for uninsured children under age 19. State Children's Health Insurance Programs help reach uninsured children whose families earn too much to qualify for Medicaid, but not enough to get private coverage. GEORGIA

CCSP--The Community Care Services Program (CCSP) helps people who are elderly and/or functionally impaired to continue living in their homes and communities. For these people, CCSP offers community-based care as an alternative to nursing home placement.

Eligible persons:

* Meet the functionally impaired criteria when assessed by a local assessment team. The assessment team works with individuals and their families in developing plans of care which are approved by the individuals’ physicians

*Would otherwise be moving into a nursing home

*Are eligible or potentially eligible for Medicaid

*Can be provided appropriate community services within state cost limits.Available Services Include:

Respite Care (RC): non-skilled, temporary relief for the primary caregivers responsible for the full-time care of a person who is frail and/or impaired.

Personal Support Services (PSS): a range of supportive services provided in the individual’s home. Personal Support Services provide light housekeeping and basic personal care.

Other services include adult day care, Alzheimer’s respite, legal assistance, advocacy, elder abuse prevention, and a pharmaceutical program for seniors that helps pay for prescription medications.
